About this role

EPIC Health Partners is growing and seeking compassionate, organized, and motivated Case Managers to join our team. In this role, you will help individuals navigate behavioral health and community support systems by connecting them with resources, coordinating services, and advocating for their needs. As a key member of the care team, you will build meaningful relationships, support individuals in achieving their goals, and help ensure access to services that promote stability, recovery, and overall well-being.


Essential Duties & Responsibilities

  • Identify individual strengths, needs, barriers, and service goals through ongoing assessment and engagement.
  • Assist with the development, implementation, and monitoring of Individual Service Plans (ISPs) in collaboration with the individual and treatment team.
  • Connect individuals to behavioral health services, community resources, healthcare providers, housing supports, employment services, transportation, and public benefits.
  • Coordinate care across providers, agencies, family supports, and community partners to promote continuity of services.
  • Maintain regular contact with individuals to monitor progress, address barriers, and support achievement of service goals.
  • Advocate for individuals and provide problem-solving support to help them access needed services and resources.
  • Facilitate referrals and assist individuals in navigating complex systems of care.
  • Participate in interdisciplinary team meetings and communicate updates regarding engagement, progress, and identified needs.
  • Complete required documentation, assessments, service notes, and reports in a timely, accurate, and compliant  manner.
  • Conduct community-based visits as needed to support engagement, service coordination, and successful outcomes.
  • Maintain compliance with all applicable confidentiality, documentation, and regulatory requirements.
  • Contribute to a collaborative team environment and support program initiatives as assigned.


Qualifications

  • Bachelor's degree in psychology, social work, counseling, vocational rehabilitation, human services, or another Virginia DMAS-approved human services field required.
  • At least one year of experience providing case management, behavioral health, substance use, or mental health services preferred.
  • Staff must be currently certified in CPR and First Aid and have verification of Tuberculosis screening.
  • Familiarity with behavioral health, substance use treatment, recovery-oriented services, and trauma-informed care principles.
  • Knowledge of DBHDS and DMAS standards, documentation requirements, and service coordination practices preferred.
  • Strong organizational, communication, and advocacy skills.
  • Ability to successfully complete required background and registry checks.
