The Clinical Liaison facilitates patient transitions from healthcare facilities to home-based skilled private duty and home health services, helping ensure continuity of care and timely coordination of referrals. Office Location: Knoxville, TNCoverage area: Knoxville, TN

Schedule: Full-Time; Days

  • Monday - Friday 8:00am - 5:00pm

Responsibilities

  • Manage communication between physicians, social workers, discharge planners, hospital case managers, and agency staff regarding referral requests and concerns
  • Relay patient care information, inquiries, and status updates to referral sources
  • Maintain a system for tracking non-admits and communicate outcomes to referral sources
  • Coordinate care needs and communicate physician and staff concerns prior to admission or resumption of care
  • Facilitate clinical orders from facilities or physicians and communicate requirements to agency staff
  • Build and sustain collaborative relationships with facility and physician staff
  • Facilitate patient referrals for home health and skilled private duty services prior to discharge
  • Conduct pre-discharge facility visits at physician request to assess patient needs and eligibility
  • Notify primary care providers or specialists of patient discharge and upcoming home health admission
  • Manage assigned accounts and identify opportunities for clinical alignment to enhance outcomes
  • Educate clinical providers on home health and skilled private duty service capabilities
  • Communicate patient acceptance and document required demographic, clinical, and payer information on approved forms
  • Support timely processing of physician orders at the branch level
  • Promote effective communication and collaboration with branch and sales staff through weekly meetings

Qualifications

  • Licensed Practical Nurse (LPN), Registered Nurse (RN), Certified Respiratory Therapist (CRT), or Registered Respiratory Therapist (RRT) licensure in the applicable state preferred.
  • Reliable personal transportation, valid driver’s license, and current automobile insurance required.
