About this role

Clinical Pharmacist Specialist will do the following:

A. Ensures safe, appropriate, cost-effective medication therapies for patients according to evidence-based medicine and following established policies, procedures, and protocols.
1. Rounds daily on all Med Surg patients and other patients as appropriate. May round individually, with Physician, or as part of multidisciplinary team as available. Provides drug information to patients, their families, and health care providers.
2. Reviews electronic medical record for each patient daily and evaluates drug therapy. Monitors drug therapy regimens for contraindications, drug/drug interactions, drug/food interactions, allergies, and appropriateness of drug and dose. Writes Pharmacist Note on each patient every day. Contacts prescriber to make changes if needed.
3. Interprets medication orders (verbal, written, or computer) and transcribes to computerized patient medication profiles accurately or verifies electronic orders.
4. Assists with pharmacokinetics consults and drug dosing per organizational protocol.
5. Performs antimicrobial stewardship activities by reviewing and interpreting culture and susceptibility data for antibiotic appropriateness and recommends changes as needed.
6. Detects and reports suspected adverse drug reactions accurately and in a timely manner.
7. Documents all clinical activities and interventions accurately and completely.
8. Participates in the quality improvement and medication use review activities of the department. Collects data, conducts quality monitors and inspections, and maintains logs, records, and other documentation as assigned.
9. Participates in the Palliative Care team and other collaborative groups as assigned.
10. Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs. Works with pharmacy and other students as assigned.
11. Performs medication reconciliations on admission and/or discharge. Verifies home medications. Checks student/intern/pharmacy technician computer entry of  Medication Reconciliations and writes related Pharmacist Notes as appropriate. Contacts retail pharmacies to arrange discharge meds as needed.
12. Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
B. Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.
1. Compounds and dispenses pharmaceuticals, including sterile, hazardous, and parenteral nutrition products accurately.
2. Issues controlled substances to patient care areas and maintains records as required by law.
C. Contributes to the quality and effective operation of the department.
1. Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel if requested.
2. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner.
3. Completes and documents inspections of medication storage areas at least monthly if assigned. Identifies and replaces outdated and unusable drugs. 
4. Answers the telephone within 3 rings, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window.
5. Keeps pharmacy areas and equipment clean, neat, and well organized.
6. Performs essential duties of Pharmacy Manager in his/her absence.
7. Follows USP 797 and USP 800 regulations for sterile compounding and handling of hazardous drugs when implemented.
8. Performs other duties as assigned.
D. Maintains competence required for current job title/position.
1. Maintains current pharmacist licensure.

2. Attends pharmacy staff meetings and other meetings as assigned.



Knowledge, Skills, and Abilities:   Ability to work independently with minimal direct supervision.  Ability to work cooperatively with health care team and pharmacy staff.  Ability to handle frequent interruptions and adapt to changes in workload and work schedule.  Ability to set priorities, make critical decisions and respond quickly to emergency requests.  Ability to exercise sound professional judgment.  Ability to communicate effectively (orally and in writing).  Ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncology, obstetric, chemically dependent, rehabilitation, and skilled-nursing patients.
Education:   Graduate of an ACPE-accredited School of Pharmacy with a PharmD degree.  (PharmD requirement may be waived for experienced internal applicants.)  Completion of PGY-1 hospital residency preferred, previous experience will be considered.
Experience:   At least 3 years of hospital experience as a clinical pharmacist preferred for applicants who did not complete a residency.  No criminal record involving drugs or theft.  A background check will be conducted.
Certification/Licensure:   Currently licensed to practice pharmacy in the state of Kansas.
Code Blue Training:   This person will be ACLS certified or complete ACLS certification by the end of the probationary period.
