About this role

The Pharmacy Department at the Dana-Farber Cancer Institute places emphasis on the values of reliability, professionalism, cooperation, teamwork, communication, positivity and inclusiveness. The Pharmacy Technician III, Medication Reconciliation is responsible for providing support to pharmacy operations under the supervision of a licensed pharmacist. The Pharmacy Technician III, will staff in pharmacy locations/areas as required.

Medication Reconciliation Pharmacy Technician Duties:
  • Practices as a pharmacy technician in accordance with all federal, state, DFCI and Joint Commission policies, regulations and standards.
  • Conduct in-person or phone meetings with patients to update medication lists prior to their provider appointments, ensuring accuracy and completeness of the medication list.
  • Create and update lists of current medications, document changes in dose, frequency, or route, and flag medications for removal to prevent harmful drug interactions.
  • Communicate drug-drug interactions or other medication changes with pharmacists and providers to maintain patient safety.  Manage medication lists for medications listed from external entities, ensuring comprehensive and accurate records.
  • Provide cross coverage in the outpatient/specialty pharmacy as needed to maintain seamless patient care. Utilizes multiple systems such as EHR (Electronic Health Record, Inventory Management Systems and other systems to complete and document duties. 
  • Troubleshoot as necessary.  Trains other employees as assigned. Communicates and escalates issues or problems. Performs other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

  • Ability to accurately perform pharmaceutical calculations.
  • Ability to utilize pharmacy technology systems.
  • Ability to develop proficiency and mastery of compounding of non-sterile products. 
  • Ability to independently troubleshoot issues and assist in resolution or escalation to the appropriate resource.
  • Ability to treat all others (fellow pharmacy staff, patients, nurses, providers, support personnel, etc.) with the utmost professionalism and respect. 
  • Ability to effectively work within a collaborative team-focused environment.
  • Participate in department meetings and functions to promote a successful and efficient pharmacy workgroup.
  • Ability to effectively communicate both orally and in writing.
  • Demonstrated critical thinking skills (ability to analyze information objectively and make a reasoned judgment).
  • Strong attention to detail. 
     

MINIMUM JOB QUALIFICATIONS:

Minimum Education:  
High School graduate or currently enrolled in an accredited entry-level degree program for Pharmacy or a related field.

Minimum Experience:
Five years of prior work as a pharmacy technician in a retail or hospital environment required.

Experience with oncology medications required.

License/Certification/Registration:
Massachusetts Board of Pharmacy Registration as a Pharmacy Technician required.  National Pharmacy Technician Certification (PTCB) must be obtained within 1 years of hire date.

Pay Transparency Statement

The hiring range is based on market pay structures, with individual salaries determined by factors such as business needs, market conditions, internal equity, and based on the candidate’s relevant experience, skills and qualifications.

For union positions, the pay range is determined by the Collective Bargaining Agreement (CBA).
