  • Boronia office, on the lands of the Wurundjeri People + hybrid working conditions
  • $46.70 – $51.26 p/Hour + 12% super + generous Salary Packaging benefits of up to $18,550


About the program and team
Each’s NDIS Support Coordination team works alongside people with disability, families, and carers to navigate the NDIS with confidence. The team supports participants to understand their plans, connect with the right services, and build the skills and knowledge to live the life they choose—at their own pace, on their own terms.

You’ll join a supportive, collaborative team that values shared learning, reflective practice, and delivering high‑quality, person‑centred support.


Your role, your impact
As a NDIS Support Coordinator, you’ll play a key role in supporting participants to move from planning to action in a way that reflects what matters to them. Your work will help participants strengthen their capacity, navigate systems with less stress, and feel more connected to their community.

You will:

  • Build strong, respectful relationships with participants, families, carers, and stakeholders
  • Support participants to understand and use their NDIS plans, building confidence and decision‑making skills
  • Coordinate services and supports that align with each person’s goals, values, and needs
  • Advocate for participants to ensure services are responsive, appropriate, and inclusive
  • Monitor plan progress, risks, and outcomes, adjusting supports where needed
  • Prepare and submit reports for the NDIA in line with plan requirements
  • Accurately record and claim support coordination hours and maintain high‑quality documentation
  • Contribute to continuous improvement, training, supervision, and team learning


About you
We’re looking for someone who brings compassion, organisation, and curiosity, and who enjoys working alongside people to solve problems and build capacity.

You’ll be well suited to this role if you have:

  • Diploma or higher qualification in a relevant health or human services discipline 
  • Experience working in disability, mental health, community, health, or human services
  • A person‑centred and strengths‑based approach
  • Strong communication and relationship‑building skills
  • Confidence managing priorities and navigating systems
  • Experience coordinating services and supporting people with complex needs
  • Ability to work respectfully with people from diverse backgrounds
  • Current state‑based driver’s licence
  • Current WWCC and NDIS Worker Screening Check (or willingness to obtain)
