Nurse Case Manager - CarepathRx - Remote
About this role
CarepathRx supports health systems and hospitals with pharmacy and infusion services designed to improve patient care.
We are seeking a Case Management Analyst (RN) to coordinate admissions and ongoing care for patients receiving infusion therapy. You will work closely with patients, providers, pharmacists, home health agencies, and internal teams.
Responsibilities
Coordinate patient referrals and admissions.
Confirm patients meet admission and clinical requirements.
Communicate with patients, providers, pharmacists, home health agencies, and internal teams.
Provide ongoing case management, including help with infusion pumps, supplies, and care transitions.
Educate patients and caregivers about therapy needs and available resources.
Deliver responsive customer service and help improve team processes.
Required Qualifications:
Graduate of an accredited nursing program.
Active Registered Nurse (RN) license in PA or Compact.
Strong communication, organization, and time management skills.
Proficiency with Microsoft Office.
Ability to manage multiple priorities in a fast-paced environment.
Preferred Qualifications:
Bachelor of Science in Nursing (BSN).
Experience in home infusion, specialty pharmacy, case management, or home health.
If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.For this position, we anticipate offering an hourly rate of 24 - 37 USD / hourly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
