- Pendidikan minimal D3 Okupasi Terapis
- Memiliki STR aktif
- Memiliki pengalaman kerja dibidang yang sama minimal 1 tahun, fresh graduate dipersilakan melamar
- Mampu bertanggung jawab dan komunikasi yang baik
