PATIENT CARE MANAGER

(PERMANENT FULL TIME)


Palmerston and District Hospital is currently accepting applications for a permanent full-time Patient Care Manager.

 

Position Overview:

Reporting to the VP Quality, Clinical Services & CNE, this position will be responsible for the operations of the Inpatient, Emergency and Surgical programs at Palmerston and District Hospital. As the leader, you are responsible for patient care, human resources, equipment, financial and overall management of the patient care areas. You will demonstrate strong leadership skills and are able to problem solve day to day issues as well as apply strategic thinking at a corporate, and regional level. Committed to excellence, you are enthusiastic and a strong advocate for continuous quality improvement and an advocate of LEAN in healthcare. You must be a strong team player able to support nursing and corporate initiatives in an inter-professional environment. You demonstrate a strong ability to collaborate and work within the organizational ethical framework and the organization's adoption of the LEADs in a Caring Environment capabilities framework to be able to provide guidance for leaders within a intricate and everchanging industry.


Responsibilities:

  • Responsible for the ongoing operations of the patient care department including patient care, scheduling, policies, procedures, documentation and standards of care
  • Responsible for financial management of clinical programs – operating & capital
  • Facilitates the clinical service committees and ensure an annual work plan is developed with key deliverables that are patient centered and related to strategic plan
  • Responsible for leadership, the implementation of best practice, the promotion of collaboration and effective communication, and promoting a just culture
  • Participates in the development of professional practice across the Wellington Health Care Alliance
  • Supports the development of the key deliverables of the strategic plan management plan, safety, quality and performance improvement initiatives, patient care and quality frameworks to build a sustainable system to ensure excellence in patient care
  • Responsibilities include human resources planning, management and recruitment, quality and risk management, performance development and Accreditation Required Organization practices and Standards
  • Participates on Regional, and local Committees as required to promote, advocate and represent the Hospital

 

Qualifications:

  • Regulated Health Care Professional in good standing with their respective College
  • Undergraduate Degree in a respective field required, Masters preferred
  • Minimum of 5 years direct clinical experience in an acute care hospital setting (rural hospital experience an asset)
  • Experience in Emergency Department or Operating Room considered an asset
  • Evidence of lifelong learning and professional development
  • Comprehensive knowledge of standards of care, professional standards, best practice, evidence-informed decision making, legislative requirements, risk management and quality improvement
  • Demonstrated excellence in communication both written and verbal
  • Demonstrated conflict management skills, project management, continuous quality improvement and change management
  • Excellent critical thinking/decision making knowledge and skills, program design, development and LEAN principles
  • Strong computer/technology skills and able to lead technology implementation related to patient care
  • Able to work independently, meet timelines, work within and lead a team, strong organizational skills
  • Able to prioritize and manage multiple priorities effectively
  • Creative, innovative and flexible

 

Additional Qualifications:

  • Excellent performance and attendance required
  • Satisfactory Reference Checks required prior to start date
  • Must provide a Police Vulnerable Sector Check
  • Must complete a Pre-Employment Health Assessment prior to being hired

 

Hours of Work:

  • 37.5 hours per week
  • The majority of the work will be completed during normal business hours, Monday to Friday
  • Occasionally attendance at meetings/hospital events during non-traditional hours (early morning, evenings and weekends) is required
  • Required to be part of the after-hours administration on-call team

 

Salary:

  • $62.94 - $73.63 per hour

 

This position requires travel amongst the three hospital sites and therefore a valid driver's license and reliable vehicle are required.
