Job Summary and Responsibilities
As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.
- Utilize the physical therapy process for planning and delivery of direct/indirect patient care.
- Identify patient/caregiver leaning needs and implement appropriate measures to meet needs.
- Assess the need for equipment, special devices. Ensure that equipment and supplies are adequate for department and patient care needs and utilized in a cost effective manner.
- Instruct patient/caregiver and/or family in prescribed therapeutic home program.
- Instruct patient/caregiver and family in the use of prosthetic, orthotic and assistive devices (canes, wheelchairs, walkers, etc.).
- Implement physical therapy care following a plan of care/treatment, and physician orders in a knowledgeable, skillful, consistent manner following agency policy, procedures, and clinical standards.
We are offering a $10,000 sign-on bonus in addition to relocation to new hires who meet the eligibility requirements.
Job Requirements
Required
- Graduate of an accredited school of Physical Therapy, upon hire and
- Physical Therapist: CA, upon hire and
- Basic Life Support - CPR, upon hire (Must be through the American Heart Association)
Preferred
- 6 months to one year home health experience
