Job Description
What You’ll Do:Â
- Assess patients to develop and initiate treatment plans based on the evaluation resultsÂ
- Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment requiredÂ
- Provide direct patient care by established protocols and patient care guidelinesÂ
- Document findings, progress, and instructions to patients and caregiversÂ
- Responsible for patient care performed by assistants and rehabilitation techniciansÂ
- Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goalsÂ
- Implement physical therapy treatment program and provide essential physical therapy treatmentsÂ
- Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companiesÂ
- Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reportsÂ
Qualifications
- Graduate from a CAPTE accredited Physical Therapy ProgramÂ
- New or recent graduates are encouraged to applyÂ
- Current state licensureÂ
- CPR certification Â
- Ability to work efficiently/effectively in an autonomous environmentÂ
- Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS OfficeÂ
