Outpatient Physical Therapist | Belmont Village St Matthews | 4600 Bowling Road, Louisville, KY 40207

 

Certification assistance available for up to $1,000, including LSVT, Geriatric, NDT, and wheelchair positioning,

Need: Part-time -20-29 hours a week. 4-5 days per week.  Would consider three full days for the right fit.

A Clinic-Style Outpatient Setting Inside a Vibrant Senior Living Community

HealthPRO Heritage is looking for an Outpatient Physical Therapist to join our team at Belmont Village St Matthews, treating active older adults in assisted living and independent living within a true outpatient Part B model. This is a calmer, more predictable alternative to a typical facility staffing role, with a consistent caseload, a clinic-like environment, and no weekend coverage.

 

Belmont Village St. Matthews is located in St. Matthews, an affluent Louisville suburb known for its strong healthcare presence, upscale shopping, and established residential neighborhoods. The area offers convenient access to hospitals, retail centers, restaurants, and downtown Louisville.  

Compensation and Perks

• Compensation: $42 to $48 per hour, based on experience
• Certification assistance up to $1,000, including LSVT, Geriatric, NDT, wheelchair positioning, and environmental modification
• Part-time benefits, including 401k, Dental, Vision, and access to free CEU's via CEU360

This role offers 20 to 29 hours per week across 4 to 5 days, with the option to structure it as 3 full days for the right fit. Responsibilities

• Patient Assessment: Evaluate patients' physical conditions, including mobility, strength, and flexibility.
• Treatment and Education: Provide therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain. Educate patients and families on exercises, posture, and body mechanics to aid recovery, prevent injuries, and offer guidance on home routines and lifestyle changes.
• Documentation: Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
• Collaboration: Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.

Qualifications

• Education: Degree in Physical Therapy from an accredited institution.
• Licensure: Valid state licensure as a Physical Therapist, or license eligible
• Skills: Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills with a patient-centered approach.
• Physical Requirements: Ability to lift and assist patients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.
