Florence’s proprietary technology platform, Nightingale, equips physicians with the data, insights, and workflows needed to succeed in risk-based care while maintaining clinical autonomy.  The Opportunity  Florence Health is seeking an experienced Registered Nurse to join our Practice Partner team and support a portfolio of independent primary care practices participating in our value-based care programs. Reporting to the Head of Network Operations, you will serve as the bridge between our provider partners and our internal product, analytics, and operations teams.  This is a role for a nurse who has already moved beyond direct patient care into population health, care management, quality, or value-based care operations. You will act as a trusted clinical resource to practices across our MSSP and ACO LEAD populations—evaluating care processes, closing gaps, and driving measurable improvement in cost, quality, and patient outcomes across populations rather than at the bedside.  Territory  This role supports a portfolio of 12–15 primary care practices across the United States. Candidates may live anywhere in the U.S.  What You’ll Do  Own a portfolio of ~12–15 independent primary care practices across our MSSP and LEAD populations
