Job Description

Reporting directly to the Director, Community Integration & Primary Health Care, the Project Manager, Central Intake is responsible for supporting the planning, implementation, and continuous improvement of projects that advance the growth, standardization, and sustainability of Central Intake and Coordinated Access pathways. The role focuses on workflow redesign, technology enablement, data-informed decision-making, stakeholder engagement, and change management across regional and provincial initiatives. 

Working closely with Central Intake leadership, internal teams, physicians, hospital and community partners, Ontario Health, and vendors, the Project Manager supports the development and implementation of standardized referral pathways, technology-enabled solutions, and process improvements that enhance access, coordination, and transitions of care. This work spans Halton Healthcare, regional partners, and broader provincial coordinated access initiatives.

Key Responsibilities

  • Lead and support projects that advance Central Intake and Coordinated Access priorities, ensuring alignment with organizational goals, provincial directives, and regional access standards.
  • Support workflow redesign, process mapping, implementation planning, and continuous improvement initiatives to strengthen referral management and patient access. 
  • Coordinate project plans, deliverables, timelines, risks, milestones, and stakeholder communications to ensure successful project execution.
  • Collaborate with internal and external stakeholders to assess needs, document workflows, and support the implementation of standardized future-state processes. 
  • Lead or support technology implementations, partner onboarding, workflow optimization, system enhancements, and issue resolution activities. 
  • Develop project documentation, presentations, status reports, training resources, dashboards, and briefing materials to support decision-making and project success. 
  • Support data collection, analysis, performance reporting, and quality improvement initiatives to drive data-informed decisions and monitor outcomes.
  • Identify, monitor, and escalate project risks, issues, and barriers, developing mitigation strategies to support successful implementation.
  • Apply change management principles to support stakeholder engagement, communication, training, adoption, and sustainability of new processes and technologies. 
  • Build and maintain strong relationships with Central Intake hubs, healthcare providers, community organizations, Ontario Health, and regional partners to advance integrated care and coordinated access initiatives. 

Qualifications

  • Bachelor's degree in Health Administration, Health Sciences, Business Administration, Health Informatics, Engineering, or a related field. Master's degree considered an asset. 
  • Project Management certification (PMP, CAPM, PRINCE2, or equivalent) and/or Change Management certification (Prosci, ADKAR, or similar) considered an asset. 
  • Minimum 3–5 years of project management experience, preferably within healthcare, health system transformation, digital health, or service delivery improvement initiatives. 
  • Experience leading or supporting complex projects involving process redesign, technology implementation, stakeholder engagement, and change management. 
  • Knowledge of Central Intake, Coordinated Access, referral management, patient flow, healthcare navigation, or integrated care models considered a strong asset. 
  • Experience working with referral management platforms, digital health technologies, or healthcare information systems (e.g., Novari, Ocean or similar solutions) is preferred. 
  • Demonstrated ability to manage multiple concurrent projects and priorities in a dynamic and fast-paced environment.
  • Strong analytical, critical thinking, and problem-solving skills, with experience using data to support decision-making, performance measurement, and quality improvement initiatives. 
  • Experience developing project plans, reports, presentations, dashboards, briefing materials, and stakeholder communications.
  • Advanced proficiency with Microsoft Office Suite, including Excel, PowerPoint, Word, Outlook, and process mapping or project management tools.
  • Exceptional communication, facilitation, presentation, and relationship-building skills, with the ability to engage effectively with clinicians, physicians, hospital leaders, community partners, vendors, and regional stakeholders. 
  • Demonstrated ability to work independently and collaboratively within cross-functional teams while fostering strong partnerships and consensus among diverse stakeholder groups.

Additional Information

Hours:  Monday – Friday – 7.5 hour days (hours subject to change based on operational needs)

Salary:  $54.219 to $67.80 per hour
