 Pay Rate:$19-$21 Hourly

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.

About the Role:

We are looking for a proactive employee to provide strength-based, family-centered, community-based services to children, youth, and families experiencing behavioral health challenges.

 

Responsibilities:

 

o   Provide individual and family-focused psychosocial rehabilitation services in home, school, and community settings

o   Assist youth in developing social, emotional, communication, problem-solving, coping, and daily living skills

o   Support youth in managing behavioral health symptoms and improving overall functioning

o   Help youth build meaningful relationships and increase community participation

o   Assist with skill development related to school success, employment readiness, self-advocacy, and independent living

o   Facilitate community integration and participation in age-appropriate activities

o   Support transition planning for youth moving between systems, programs, levels of care, or developmental stages

o   Collaborate with families, care coordinators, behavioral health providers, schools, DSS, juvenile justice, health care providers, and community organizations

o   Participate in Child and Family Team meetings and other interdisciplinary planning meetings

o   Assist families in identifying and accessing community resources and supports

o   Promote family engagement and youth voice throughout service delivery

o   Implement services consistent with SAMHSA Systems of Care principles and values

o   Utilize trauma-informed, culturally responsive, and strength-based practices

o   Maintain knowledge of evidence-informed practices, child-serving systems, and community resources

o   Provide person-centered, quality, services

o   Demonstrate effective time management skills

o   Observe confidentiality and safeguards regarding all participant related information

o   Follow Agency standards and procedures

o   Maintain compliance with applicable program rules and regulations

o   Contribute to department and Agency quality improvement initiatives

o   Ensure compliance with federal, state, and local employment laws and regulations, as well as with company policies

o   Perform Other Duties as Assigned

 
 

Education/Experience:

High School Diploma/GED and four years of relevant experience working with children, youth, and families in behavioral health, education, child welfare, juvenile justice, or a related field, required. Associate degree in Human Services, Social Work, Psychology, Counseling, Education, or a related field and two years of relevant experience working with children, youth, and families in behavioral health, education, child welfare, juvenile justice, or a related field, preferred. Bachelor's degree in Human Services, Social Work, Psychology, Counseling, or a related field, preferred. Experience providing PSR services and working within Children's Health Home, Systems of Care, Care Management, or other community-based behavioral health programs, preferred. Candidates must have experience supporting children and adolescents with behavioral health challenges, knowledge of child and family serving systems and community resources, and strong interpersonal, engagement, and communication skills.

Conditions of Employment:

Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs. Responsible for recertifying and maintaining licensure. Must respect confidentiality. Must be able to perform duties with repetitive motion. Must have a moderately flexible schedule. Must be available for occasional early morning, late evening, or weekend, functions; and on-call for bona fide emergencies.
