Job Description
- Perform true case management functions, interacting with patient and managing their clinical care telephonically
- Work with patients/members in the foster care program
- You will be responsible for developing and assessing the care plan and promote desired outcomes
- Provider patient and provider education, and more!
Qualifications
- RN license clear and active in the state of Washington
- Case Management experience
- Pediatrics experience preferred
- 2+ years of acute/inpatient hospital experience
- Strong Computer Skills
