What You’ll Do

As an RNAC, you’ll be a key driver of quality care and compliance. Your impact will include:

  • Take part in MDS & Care Planning ProcessEnsure accurate, timely completion and compliance with federal, state, and facility standards.

  • Collaborating Across TeamsAct as the liaison between nursing and interdisciplinary team members to ensure seamless care coordination.

  • Driving Clinical ExcellenceEvaluate Residents for significant changes, initiate assessments, and support Medicare eligibility determinations.

  • Educating & Empowering StaffProvide ongoing education and guidance on MDS processes and care planning best practices.

  • Ensuring Compliance & AccuracyMaintain documentation integrity, coordinate submissions, and track Resident status updates with regulatory agencies.

  • Supporting Quality ImprovementParticipate in Quality Assurance initiatives, committee work, and ongoing care improvement efforts.

  • Stepping in When It Matters MostProvide hands-on nursing support when needed to ensure Residents receive the highest level of care.

What You Bring

✅ Active Pennsylvania Registered Nurse (RN) license✅ Knowledge of OBRA and Department of Health regulations✅ RNAC experience or MDS/Resident Assessment Certification (required)✅ Strong attention to detail, collaboration, and communication skills

  • PRN / As-needed flexibility

  • 100% on-site (non-remote)
