POSITION SUMMARY

The Service Navigator helps patients access the services, supports, and resources they need to support their treatment and overall well-being. This position coordinates care and connects patients with mental health, substance use, physical health, recovery, and community resources while helping address barriers such as transportation, medication access, and appointment scheduling. The Service Navigator works closely with patients, families, healthcare providers, hospitals, and community partners to promote continuity of care and ensure patients receive appropriate support throughout their treatment.

 DUTIES AND RESPONSIBILITIES

1. Provide appropriate linkages, referrals, coordination, and follow-up for patients to services and supports, including referrals and linkages to long term services and supports.
2. Communicate with internal and external providers to ensure that patient needs for supports and linkages align with basic needs and treatment goals.
3. Provide health care appointment scheduling for patients to mental health, substance abuse, and physical health providers.
4. Ensure patient has adequate transportation to and from appointments, including transporting patients directly as needed.
5. Participate in hospital discharge processes and communicate with other providers and family members.
6. Communicate and collaborate with hospital emergency department staff to ensure the coordination of patient care and treatment.
7. Assist patients in obtaining necessary medications and increasing medication compliance.
8. Work closely with community resources and organizations in coordinating patient services.
9. Advocate for patients who may be vulnerable or need additional support.
10. Complete and file all documentation as required in a timely manner.
11. For grant funded patients, complete GPRA and all other grant required documentation as required by contractual requirements.
12. Connect patients with recovery supports as resources are available.
13. Ensure releases of information and other required documents are completed and filed.
14. Assist with Quality Improvement efforts.
15. Treat all individuals with dignity and respect while maintaining agency standards of confidentiality, professionalism, and objectivity.
16. Maintain all HIPAA and 42 CFR confidentiality requirements and regulations as outlined in CARF policies and procedures.
17. Comply with agency policies, procedures, and regulatory standards.
18. All other duties as assigned.

QUALIFICATIONS/SKILLS/KNOWLEDGE REQUIRED FOR POSITION

1. Bachelor’s degree in human services field or 3 years of relevant experience working with adults and/or children diagnosed with multi-occurring disorders.
2. Supportive of the mission of Prairie Ridge and the patients we serve.
3. Proficiency in oral and written communication skills, as well as documentation requirements.
4. Technology skills allowing for proficiency in use of electronic health record.
5. Travel may be expected.
6. While performing the duties of the job, the employee is regularly required to sit; use hands to finger, handle, or feel objects, tools, or controls; and talk or hear. The employee frequently is required to walk and stoop, kneel, crouch, or crawl. The employee is often required to stand, reach with hands and arms, and climb or balance.
7. The employee must infrequently lift and/or move up to 10 pounds and occasionally lift and/or more up to 35 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, and the ability to adjust focus.
