Responsibilities

Resident Support & Advocacy

  • Complete psychosocial assessments and identify residents’ emotional, social, and psychological needs
  • Develop and implement individualized care interventions that support resident goals and quality of life
  • Provide support and guidance to residents as they adjust to the nursing home environment
  • Advocate for resident rights, dignity, independence, and personal choices
  • Provide emotional support during challenging transitions and life events
  • Assist residents and families with accessing appropriate community resources and services

Care Coordination & Interdisciplinary Collaboration

  • Participate as an active member of the interdisciplinary care team
  • Collaborate with nursing, rehabilitation, dietary, administration, and other departments to support resident-centered care
  • Participate in care planning meetings and quality improvement initiatives
  • Assist with discharge planning and coordination of appropriate community services
  • Provide education and support to residents, families, and caregivers regarding available resources and services

Documentation & Compliance

  • Maintain accurate and timely documentation related to resident assessments, interventions, and progress
  • Ensure compliance with applicable state and federal regulations and professional standards
  • Support facility initiatives related to resident rights, quality improvement, and person-centered care
  • Maintain confidentiality of resident information in accordance with HIPAA regulations

Qualifications

Qualifications

  • Bachelor’s degree in Social Work or related field required (based on state requirements)
  • Current Social Work licensure or certification required as applicable by state regulations
  • Experience working with older adults, long-term care, rehabilitation, or healthcare populations preferred
  • Knowledge of psychosocial needs, care planning, and resident rights
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
