Job Summary and Responsibilities
As our Social Worker, you will help residents in Skilled Nursing Care perform social work screenings and interventions so they can coordinate discharge planning, assure their medically related emotional and social needs are met, and transition safely with appropriate community resources.

Every day you will plan, organize, develop, and direct the operation of Social Services in accordance with regulatory standards, interview new residents, and complete timely and comprehensive social assessments and progress notes.

To be successful in this role, you must possess a Bachelor's degree in Social Work, have at least one year of supervised social work experience in a healthcare setting, and demonstrate strong abilities in discharge planning, crisis management, and interdisciplinary collaboration.
- Interview all new residents within five days of admission, obtaining pertinent social history and initiating a resident care plan entry.
- Complete timely and comprehensive progress notes and Quarterly assessments specific to residents' social service needs and problems.
- Attend all care planning meetings for assigned residents, providing psychosocial information and participating in the development and implementation of Social Service policies.
- Participate in quality assurance activities for the department, identifying trends or concerns and developing improvement plans.
- Refer residents and families to appropriate social service agencies when facility services are not sufficient, providing written information.
- Attend and participate in Resident and Family Council meetings, addressing concerns and assisting in developing written social work treatment plans.
$5,000.00 Sign-On Bonus offered for qualified new hires. Per policy, current employees are not eligible.Â

Job Requirements
Required
- Bachelor of Science in Social Work from an accredited program
- One (1) Year Of Supervised Social Work with residents in a healthcare setting
- Basic Life Support – Cardiopulmonary Resuscitation (BLS-CPR)
Preferred
- Skilled Nursing background
- Social Worker with 1 year of skilled nursing facility experience and expertise in discharge planning.
