Job Description

Are you an experienced Registered Nurse looking for a new opportunity to get into Telephonic RN Case Management or Patient Care Coordination? The Registered Nurse Care Coordinator is responsible for quality and continuous improvement within the job scope.

• The Registered Nurse Care Coordinator is responsible for all actions/responsibilities as described in company controlled documentation for this position.

• Contributes to and supports the corporation's quality initiatives by planning, communicating and encouraging team and individual contributions toward the corporation's quality improvement efforts.

• The Registered Nurse Care Coordinator is responsible for comprehensive patient assessments through health care records, physician and patient interviews.

• Develops, implements, and evaluates comprehensive patient care plans.

• YOU are responsible for collaboration with healthcare team to provide feedback, updates and recommendations regarding health status

• and plan of care for patient.

• The Registered Nurse Care Coordinator brings closure to the care and/or episode of illness by discontinuing case management when the highest level of function or best

• possible outcome has been obtained or the needs of the patient change.

• Obtains Hospital/SNF/Specialist and other medical records when needed.

• YOU Submit admissions and discharges e-mail notifications.

• Documents all care coordination communications and interventions.

Qualifications

• RN license in the state of NV

• 2 years of experience would suffice for this position

• Must have strong computer skills

• Experience creating care plans for patients

• Strong assessment skills

Hours for this Position:

Monday – Friday 8am – 5pm
