Home Health Registered Nurse (RN)

Overview

We are seeking a Full-Time Registered Nurse (RN) for a rewarding home health position providing skilled nursing visits to adult and geriatric medical/surgical patients in their homes. This role functions as a case manager, allowing you to build meaningful relationships with patients while enjoying flexibility and independence in your day.

Coverage Area

Patients are located throughout the Cleveland metropolitan area, including portions of the East Side, Near West Side, and surrounding communities. We work closely with clinicians to build efficient schedules and minimize drive time whenever possible.

Responsibilities

• Perform skilled nursing visits in patients' homes• Manage and coordinate patient care plans• Complete clinical documentation and assessments• Conduct supervisory visits as directed• Collaborate with the clinical team to identify opportunities for improved patient outcomes• Promote high-quality patient care and service excellence

Requirements

• Active Ohio Registered Nurse (RN) license• At least 1 year of recent experience with adult and geriatric medical/surgical patients• Valid driver's license and reliable transportation• Comfortable working independently in the field• Authorized to work in the United States; employer-sponsored work visas are not available for this position
